Jordan's Principle Case Manager

Position Summary:
Reporting to the Health Director and Chief and Council, the Case Manager oversees and administers all aspects of the Jordan's Principle Program, operating according to the policies and procedures of the Birdtail Sioux Health Centre. Case Management is the primary responsibility, identifying and assessing the needs of children and youth (0-18 years) and their families and coordinates services to be offered to meet their required needs. The components of services and support must be related to health, social and educational services. The primary focus is to provide services in a timely manner, without unnecessary delays.
The Case Manager supervises all staff under Jordan's Principle, performing all management and human resource functions.

Key Responsibilities:

Service Delivery:
- Use a family centered, strength-based approach to assess parent/child attachment, family challenges, strengths and needs, connection to community resources, and existing personal and professional support advocating for services that promote and protect the health and well-being of clients, care givers and family members.
- Coordination and workflow process from intake, ongoing case management and discharge in conjunction with programs.
- Manage service-related data collection processes.

Administrative Functions:
- Managing, supervising and evaluating staff, scheduling work assignments etc., ensure proper case management is adhered to, ie. Organizing notes, follow up with clients, ensuring client documentation is organized in a central location.
- Work in collaboration with assigned finance department staff to ensure that all financial claims related to Jordan's Principle are processed and managed according to policy and procedures.
- Formalize monthly reports, attendance records and travel expense claims.
- Complete and submit ISC reporting.

Community Supports:

- development, planning, delivery and evaluation of treatment, education, counselling, and mental health promotion programs for the community.

Qualifications/Education/Skills:
- Post-Secondary degree (BN/RN, LPN, BSW, BEd.) or equivalent combination of education, training and experience.
- 3 years' experience working with First Nations communities, specifically with children and youth with developmental and intellectual disabilities.
- Clear Criminal Records Check, Vulnerable Sector Check and Child Abuse Registry Check.
- Personnel management experience.
- Knowledge of Dakota Language and Traditions.
- Strong organizational and problem-solving skills.
- Proficiency in financial software and Microsoft Office applications.
- Ability to work collaboratively with leadership and program managers.
- Ability to work independently and as part of a team.
- Attention to detail and problem-solving skills.
- Excellent written and verbal communication abilities.
- Friendly and professional demeanor.

Working Conditions:
- The Case Manager will typically be in an indoor setting and is required to operate and maintain all First Nation office equipment.
- The physical demands include, but are not limited to, standing, sitting, walking, lifting, carrying, reaching, handling, kneeling, crouching and bending.
- Non-physical demands include a work environment where the noise level is usually quiet to moderate but may be loud on occasion.
- Occasional evening, weekend, or emergency response duties.
Terms of Employment:
- Duration: Full-time
- Working Hours: Full-time, Monday to Friday 9:00 AM - 4:30 PM
